Thomas R. Leonard, M.D.
Primary Duty: Physician Dr. Thomas Leonard has been serving Carthage and the Moore County area since 1998. He appreciates the variety of challenges associated with family medicine and enjoys treating patients of all ages. In his free time, he enjoys playing basketball, singing in his church choir and participating in various activities with his wife and two children. He is an avid Indiana University Basketball fan. Shawana Faulk, FNP
Primary Duty: Family Nurse Practitioner. Moore Family Medicine, P.A. is pleased to introduce our newest provider, Shawana Faulk, FNP. Shawana joins our practice as a Family Nurse Practitioner. She received her associate nursing degree at State University of New York and recently earned her FNP degree from the University of Phoenix. Shawana is married and has 4 children. Shawana specializes in Cardiology and Women’s Health. She is a member of the North Carolina Nurses Association (NCNA) and the American Academy of Nurse Practitioners (AANP). Patricia Bartley, LPC
Primary Duty: Licensed Counselor: Patricia Bartley joined Moore Family Medicine in January, 2016 to provide behavioral care and counseling services. She received her B.A in psychology at Rutgers University, and MS in special education at Syracuse University and an M.A in community counseling at Campbell University. Patricia has a broad range of experience in treating patients of all ages with a variety of behavioral conditions. An appointment with Ms. Bartley can be requested by calling 947-3000.
