For the first time Moore Family Medicine will be hosting Flu Shot Clinics.

In the coming weeks we will post clinic dates and times that you and your family can come in without an appointment to get your flu shot.

Most insurances will cover the full cost of the vaccine. We are offering the vaccine for $25 to our uninsured patients.

An announcement will be made once our vaccines arrive. This season get your flu shot and not the flu!

Below are the forms that Moore County Schools requires for Health Assessments and Sports Physicals.  Please print out your child’s required form prior to their appointment.  The Screening for Vaccine Contradictions form is required for any child receiving a vaccine.
